Basic setup for Survey Template

1. Navigate to https://app.edcforge.com/dashboard

2. Click "BRE / Surveys"

3. Click "Add New Template"

4. Click "Template Name" field.

5. Click "Survey Category" options.

6. Click this button.
Select "Question types"
Click "Save"
