Understanding Field Capture in the Survey Builder

1. Navigate to https://app.edcforge.com/dashboard

2. Click "BRE / Surveys"

3. Click "Templates"

4. Click "Add New Template"

5. Click "Template Name" field.

6. Click "Survey Category"

7. Click this icon.

8. Click the "Single Answer" field.
Click "Save"

9. Click the "Enter question text" field.

10. Click this checkbox.

11. Click "Record Type".
Click "Companies"

12. Click "Map With"

13. Click "Save".
